Two set of procedures are prescribed for following two categories:

Category 1

a) A woman applying for change of surname following marriage;
b) Divorcees/Widow applying for change from married name to maiden name;
c) Remarried applicants applying for change of surname

Category 2 -All other cases not covered in Category 1 

Requirements ( for Category  1)

1  Applicants are required to fill online application form at https://portal5.passportindia.gov.in  and carry a printout of the filled in application form. 
2 a  Marriage certificate or Joint Photo Declaration by both husband & wife (if surname change is based on marriage)
2 b   Divorce order/decree (if name/surname change is based on divorce)
2 c   Divorce order/decree or death certificate of the first husband and remarriage certificate or Joint Photo Declaration by both husband & wife  (in case change of surname after remarriage)

3   Affidavit regarding change of surname after marriage/divorce 
4   Copy of  Passport/Aadhar ID of husband in case of marriage/remarriage
5  Three recent passport-size photographs of 2 inch x 2 inch with white background.
6  Original Passport (including subsequent additional booklet(s), if any).
7  Photocopy of Passport (first two and last two pages and all pages of observations, if any, made by Passport Issuing Authority)
8  Copy of valid Iqama (original to be shown for verification).
9  Personal presence of the applicant at the Application Centre is a must.
10  Prescribed fees.

12 TATKAL FACILITY IS NOT AVAILABLE FOR THIS TYPE OF APPLICATIONS.
Note : Marriage Certificate/Divorce order/decree originating from India should be attested by concerned State Home department and Ministry of External Affairs, New Delhi or its Branch Secretariats

2  Three recent passport-size photographs of 2 inch x 2 inch with white background.
3  Original Passport (including subsequent additional booklet(s), if any).
4  Photocopy of Passport (first two and last two pages and all pages of observations, if any, made by Passport Issuing Authority)
5  Copy of valid Iqama (original to be shown for verification).
6  Personal presence of the applicant at the Application Centre is a must.
7  Prescribed fees.

9   TATKAL FACILITY IS NOT AVAILABLE FOR THIS TYPE OF APPLICATIONS.
10   At least two public documents issued in the desired/ applied changed name to ascertain that the applicant has actually changed his name.
11   Original newspaper cuttings announcing the change of name published in two leading daily newspapers (one popular daily newspaper in Saudi Arabia and another at his permanent address in India).  Gazette notification published in the Indian Union/State Official Gazette can be a substitute for advertisement in Indian newspaper.

Advertisement format for Minor Applicant:

I (name of father), F/o _____________ (Given name) and _________________ (Surname) R/o_______ (address in India), holding Passport No. _____________issued at ___________ on _____________ hereby announce to change my minor son's/daughter's name as _____________ (Given name) and _________________ (Surname) for all future purposes.

Advertisement format for Major Applicant:

I _____________ (Given name) and _________________ (Surname), S/o _____________ (name of father), R/o_______ (address in India), holding Passport No. _____________issued at ___________ on _____________ hereby announce to change my name as _____________ (Given name) and _________________ (Surname) for all future purposes.
